It's a really exciting time to join us as we begin to deliver the Heathier You NHS Diabetes Prevention Programme (NDPP) throughout Cornwall, and we're looking for a group of passionate Health Coaches (or Diabetes Prevention Practitioners!) to join us on a casual contract basis as part of our bank team!The purpose of a health coach is to improve lives, through empowering people to make effective long-term changes that halt or reverse the progression of Type 2 Diabetes. This is achieved through successfully delivering our quality programmes and providing evidence-based information, supporting individuals to improve their health, lose weight and ultimately lower their risk of developing diabetes.We are seeking to provide county-wide coverage of this programme throughout Cornwall, therefore we're looking for flexible practitioners who can deliver the service when referrals are high.Like all roles at Thrive Tribe, you can expect your time at work to be fun, varied and challenging. We don't like to stick to a firm JD as we know from experience that things change throughout the lifetime of a role and it gives our people the chance to play to their strengths.A typical day as a Health Coach might include:<ul><li>Delivering evidence-based interventions that focus on diet, nutrition, physical activity and behaviour change</li><li>Facilitating group intervention sessions to support your clients in achieving their goals and cultivating a safe and positive environment</li><li>Promoting the Healthier You NHS Diabetes Prevention Programme internally and externally, and improving referral pathways between other services and partner organisations</li><li>Engaging in ongoing training relevant to your role, such as Behaviour Change Level 1 and 2</li><li>Working towards criteria and targets established within the National Diabetes Prevention Programmes (NDPP) quality assurance framework, including all Clinical Governance procedures</li><li>Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments - we're always looking to share knowledge and resources!</li><li>Ensuring due consideration and adherence to our internal Thrive Tribe policies, including Information Governance, Safeguarding and Complaints procedures</li><li>Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!</li></ul>About you More than anything, we are looking for a team player who puts their heart into their work. BDS are currently recruiting for a Support Worker to join a Husing Association in the Redruth area to support a servicer which provides support to customers, living in the community, who have severe and enduring mental health diagnoses. We provide social inclusion support to these customers, to enable them to have a better quality of life.
